This is the service according to the 1916 Book of Worship of the Evangelical Synod of North America.
Dearly Beloved: Our blessed Saviour Jesus Christ instituted the Sacrament of holy Communion, that it might be the abiding memorial of His atoning death; the seal of His perpetual presence in the Church through the Holy Spirit; the mystical representation of the sacrifice of Himself on the cross; the pledge of His undying love to His people; and the bond of His loving union and fellowship with them to the end of time.
The celebration of the Lord's Supper has ever been regarded by the Church as the innermost sanctuary of the whole Christian worship. We have to do here, not with outward signs merely, but with heavenly realities which these signs represent. The Lord's Table can, therefore, be rightly approached only by those who are of a devout, repentant and believing mind. This holy mystery is not for the irreverent or indifferent.
If any of you, then, are conscious that you are the willing servants of sin, that you are without true repentance and faith, we solemnly warn and admonish you that ye presume not to come to the Lord's Table. For those doing so, eat and drink judgment to themselves; not because they are sinners, but because they are impenitent; not because they are unworthy, but because they eat and drink unworthily, not discerning the Lord's body.
On the other hand, we cordially invite all to partake of this Sacrament who are truly grieved and penitent for their sins, who look to Jesus Christ for righteousness and salvation, who abide in the fellowship of His Church, and who desire to possess His Spirit and walk in His ways. To all such the compassionate Redeemer Himself says: Come unto me all ye that labor and are heavy laden, and I will give you rest.
Ye, then, who have earnestly searched your own hearts, and who are truly penitent and desire to forsake all sin and follow after Christian holiness, as required by the Word of God, approach with me now to the throne of grace, and (upon bended knees) make your humble confession to Almighty God.
ALMIGHTY God, Father of our Lord Jesus Christ, I, a poor sinner, acknowledge and bewail my manifold sins and wickedness, which I have from time to time committed against Thy divine Majesty by thought, word and deed, from unbelief, ingratitude, unfaithfulness, and want of brotherly love, which have marred all my life, provoking against me most justly Thy wrath and indignation, in this world and in the world to come. I do earnestly repent, and am heartily sorry for these my transgressions, the remembrance of them is grievous unto me; their burden is intolerable. I have no other comfort or hope than Thy grace which aboundeth above my guilt, and the precious merits of my Lord Jesus Christ. Longing after this grace I say: Father, I have sinned against heaven, and in Thy sight, and am no more worthy to be called Thy child; I come, however, at this time of grace to ask of Thee pardon and peace, new confidence toward Thee, and strength to lead a new and righteous life through Thy Holy Spirit. Amen.
The Communicants then shall remain standing, (or if kneeling, they shall rise) and the Minister shall say:
I now ask you before the omniscient God, and upon the evidence of your own conscience:
Do you acknowledge and deplore your many sins with a contrite heart?
Do you truly believe that our heavenly Father is willing, for Jesus' sake, to forgive all your sins?
Do you fully resolve to submit yourself in future to the gracious direction of the Holy Spirit, so that you may no more purposely sin, but be enabled to follow after holiness?
If so, make it manifest by saying: I do.
Answer: I do.
The Minister shall proceed as follows:
Upon this humble confession which you have made, as a Minister of our Lord Jesus Christ, and by His authority, I declare unto you, who do truly repent and heartily believe in Jesus Christ, and are sincerely determined to amend your sinful life, the forgiveness of all your sins in the Name of the Father and of the Son and of the Holy Spirit. Amen.
But be it known unto all such as are impenitent and unbelieving, that their sins are retained; and in Christ's stead I earnestly admonish them to repent, to believe in the Gospel, and to be reconciled to God. May the love and mercy of God lead them to sincere repentance and true faith. Let us pray:
ALMIGHTY and eternal God, we have sinned against Thee, and because of our sins have merited everlasting condemnation. But, as we believe Thy beloved Son, our Lord, hath wrought for us everlasting salvation, and having the assurance of the forgiveness of our sins in Thy holy Word, and of absolution, we humbly beseech Thee to fill us with the power of Thy Holy Spirit, that we may ever guard against sin, and in our holy calling live a truly pious life, through Jesus Christ our Lord. Amen.
Let us hear with reverent hearts the words of Christ instituting this Holy Supper.
Our Lord Jesus Christ in the night in which He was betrayed took bread; and when He had given thanks, He brake it, and gave it to His disciples, and said: Take, eat; this is my body, which is given for you; this do in remembrance of me.
In like manner also He took the cup, after supper, and when He had given thanks, He gave it to them, saying: Drink ye all of it; this cup is the New Covenant in my blood, which is shed for you, and for many, for the remission of sins; this do, as oft as ye drink it, in remembrance of me.

PRAISE, and honor, and glory be unto Thee, O Christ! The bread which we bless is the Communion of Thy holy Body, and the cup which we bless is the Communion of Thy holy Blood. O Thou everlasting Son of the Father, sanctify us by Thy Holy Spirit, and make us worthy partakers of Thy sacred Body and Blood, that we may be cleansed from sin and made one with all the members of Thy Church in heaven and on earth. Graciously hear anew the resolution of our hearts: Lord Jesus, for Thee we live, for Thee we suffer, for Thee we die, Thine will we be in life and death. Grant us, O Lord, eternal salvation.
OUR FATHER, who art in heaven; hallowed be Thy Name; Thy kingdom come; Thy will be done on earth, as it is in heaven; give us this day our daily bread; and forgive us our debts, as we forgive our debtors; and lead us not into temptation; but deliver us from evil; for Thine is the kingdom, and the power, and the glory for ever. Amen.

O Lamb of God who, bleeding,
Upon the cross didst languish,
Nor scorn nor malice heeding,
So patient in Thine anguish,
On Thee our guilt was lying;
Thou savedst us by dying:
Have mercy on us, Lord Jesus.
The Minister shall then say:
Come unto me all ye that labor and are heavy laden, and I will give you rest. Thus our Lord invites all who truly repent and heartily believe, and vouchsafes to them His forgiveness and grace. Come, for all things are prepared. Taste and see how good the Lord is. Blessed are they who trust in Him. Amen.
When the Communicants come to the altar, the Minister shall give the Bread, saying:
Take and eat, this is the Body of our Lord Jesus Christ, which was given for you; this do in remembrance of Him.
When the Minister gives the Cup, he shall say:
Take and drink, this is the Blood of our Lord Jesus Christ, which was shed for you, and for many, for the remission of sins; this do in remembrance of Him.
or
Take and drink ye all of it; this is the Cup of the New Covenant in the Blood of Christ, which was shed for you, and for many, for the remission of sins.
After those standing at the altar have communed, the Minister shall dismiss them, saying:
May this strengthen and preserve you unto everlasting life. Go ye in peace. Be it unto you according to your faith. Amen.
When all have communed, the Minister shall say:
Oh give thanks unto the Lord; for He is good; for His lovingkindness endureth for ever. (Ps. 136:1)
Bless the Lord, O my soul; and all that is within me, bless His holy Name. Bless the Lord, O my soul, and forget not all His benefits: who forgiveth all thine iniquities; who healeth all thy diseases; who redeemeth thy life from destruction; who crowneth thee with lovingkindness and tender mercies. Bless the Lord, O my soul.
ALMIGHTY and everlasting God, we most heartily thank Thee, that Thou didst vouchsafe to strengthen us through this holy Sacrament. We praise Thee, that Thou dost assure us thereby of Thy grace in Jesus Christ. We rejoice that through this Sacrament our communion with Christ and all believers is strengthened, and that we have become heirs of Thine everlasting kingdom, by the merits of the death of Thy dear Son. We humbly beseech Thee, O heavenly Father, so to assist us by the grace of Thy Holy Spirit, that we may continue in this holy fellowship, and live and labor according to Thy holy will; through Jesus Christ our Lord, to whom, with Thee and the Holy Spirit, be all power, honor and glory, world without end. Amen.

The Lord make His face to shine upon thee, and be gracious unto thee:
The Lord lift up His countenance upon thee, and give thee peace. Amen.
